As artificial intelligence becomes more deeply embedded in our daily workflows, the way we interact with information is undergoing a profound transformation. The next frontier isn’t just about smarter algorithms or faster automation—it’s about establishing “fact control”: a collaborative, dynamic process where both humans and AI curate, verify, and update a single, living source of truth. This concept is central to what I call a cognitive operating system—a new paradigm for how organizations and individuals manage knowledge, make decisions, and drive innovation.
The Problem: Fragmented Truth in a Hyperconnected World
Today, for the majority of companies information is everywhere, but truth is next to impossible to find. As I've previously written, in most organizations, enterprise knowledge is a chaotic, self-contradictory mess of disconnected documents, fragmented files, and siloed concepts. You can read more about that here
— FUBAR Mode: Why Businesses Are Getting AI Wrong
Teams work from different versions of documents, data is siloed across platforms, and AI models are only as good as the (often outdated or conflicting) data they’re trained on. The result? Misinformation, misalignment, and inaccurate AI output. It's no surprise that in a recent MIT study found 95% of enterprise AI pilots fail. They are built on a foundation of sand, or more a stinking swamp of knowledge.
Most businesses are only at a very basic implementation of AI, referencing documents of knowledge bases with RAG. RAG (Retrieval-Augmented Generation) is an AI technique that combines real-time information retrieval from knowledge bases with generative models. RAG struggles with disconnected knowledge because it can only retrieve and generate answers from what’s available—so if information is fragmented, inconsistent, or siloed, the AI’s output becomes incomplete, contradictory, or misleading.
Simply making models bigger doesn’t solve the problem of disconnected knowledge—large models can memorize more data, but they still can’t reason across gaps, contradictions, or silos. True accuracy requires structured, connected knowledge—not just more parameters.
A lot of focus has been on how memory and larger context windows can be of use to solve this problem. But that in itself is misleading, business aren't built on memory that's history and everything that every happened. Businesses are built on facts that change over time, your product pricing is a fact, your quarterly goals are a fact, your brand positioning is a fact.
AI can revolutionize business, and we're seeing that at NOAN, but only when built on a unified, continuously updated source of truth is more urgent than ever. But achieving this isn’t just a technical challenge—it’s a cultural and operational one. It requires a new approach to how we create, manage, and interact with facts.
What is Fact Control?
Fact control is the process of collaboratively managing the lifecycle of facts—curating, verifying, updating, and contextualizing them—so that every stakeholder, human or AI, operates from the same foundation. It’s not about rigidly locking down information, but about creating a living, breathing knowledge system that evolves as new insights emerge.
Key Elements of Fact Control
- Collaborative Verification: Facts are shaped by the collective intelligence of humans and AI. Discrepancies are flagged, debated, and resolved in real time.
- Dynamic Updating: As new data becomes available, the source of truth is updated—automatically by AI, or manually by human experts—ensuring relevance and accuracy.
- Contextual Awareness: Facts are linked to their sources, context, and history, making it easy to trace how knowledge has evolved and why certain decisions were made.
Fact Control in Action: The Cognitive Operating System
Fact control is a foundational layer of the cognitive operating system—a concept I’ve explored in depth here. Unlike traditional operating systems that manage hardware and software, a cognitive operating system orchestrates knowledge, context, and intent across every aspect of an organization.
How Fact Control Powers the Cognitive OS
- Continuous Learning: AI agents and humans co-edit the truth, making the system smarter and more adaptive with every interaction.
- Seamless Alignment: Teams, tools, and processes stay in sync, reducing friction and accelerating decision-making.
- Ethical and Explainable AI: Transparent fact management helps mitigate bias, ensures accountability, and makes AI-driven decisions explainable and auditable.
The Benefits: Why Fact Control Matters
- Trust at Scale: In an era of deepfakes and misinformation, fact control becomes the backbone of trust, transparency, and accountability.
- Faster, Better Decisions: With everyone working from the same facts, organizations can move faster and with greater confidence.
- Resilience and Adaptability: As the world changes, so does the knowledge system—helping organizations adapt in real time.
Implementing fact control is the most important aspect of building a business with AI. Fact control is at the heart of NOAN’s approach to knowledge management. NOAN empowers users to collaboratively curate, verify, and update a living source of truth. By combining human insight with AI-driven verification and contextualization, NOAN ensures that every decision, workflow, and strategy is built on accurate, up-to-date knowledge. Facts are stored and managed by humans and AI in NOAN's Smart Blocks. This dynamic fact control not only reduces misinformation and misalignment but also creates a transparent, auditable foundation for smarter business growth—making NOAN a true cognitive operating system for the modern organization.
The Road Ahead
As AI becomes more deeply woven into the fabric of our lives, fact control will be the linchpin for reliable, context-aware, and ethical collaboration. The future isn’t just about smarter machines—it’s about building a shared cognitive foundation where truth is a living, collective asset.
